3. The Vademecum Questionnaire
You are required to enter your first name, surname, country and a way of contacting you in case there is a query regarding the content of your questionnaire. Any other personal information in the questionnaire is voluntary. If the contents of your questionnaire are made available on the website, we will add your name and country unless you explicitly ask for this information to be withheld. Any other information such as your place of residence or contact data will be used exclusively by the Vademecum editors and not be passed on to third parties. Names of other persons mentioned in the text (patients, relatives, therapists, etc.) will be anonymized before publication.
